1. Moth balls have chemical called naphthalene or para- dichlorobenzene. This chemical has be reported to be poisonous and can lead to toxicity.
Napthalene destroys red blood cells (causes hemolysis) or result in oxidative stress resulting in methhemoglobinemia. As from the results the amount of hemoglobin is found to be down and the mean cell volume is also reduced, this indicates at hemolysis of red blood cells after ingestion of naphthalene . The diagnosis will be toxicity due to ingestion of naphthalene.
2. Glucose 6- phosphate dehydrogenase is an enzyme present in cell membrane of RBCs ( red blood cells). The enzyme is essential in phosphate pentose pathway which can protect the cell in case of oxidative stress. However deficiency of this enzyme can make cells very susceptible to the oxidative stress. Thus in case of above situation, the pathway that is hindered is the one that protects cells from oxidative stress. It can be pentode phosphate pathway due to absence of the G6PD enzyme.
3. The metabolites of naphthalene are compounds such as alpha-naphthol which can cause oxidative stress. Since RBCs carry hemoglobin, a molecule adapted to carry oxygen, any kind of oxygen stress can impact hemoglobin and the cells carrying it drastically in comparison to other cells.
4. Bilirubin is found in red blood cells. When they die, it is released into blood from where the liver filters it. The high amount of bilirubin in blood is indicative of death of Red blood cells.
5. Usually the kidney can filter any cells or proteins from the blood. This means that in case of healthy people they are not present in urine. So probably in above case the kidneys are still functioning properly and hence the RBCs and hemoglobin are not detected in urine. It could be early stage of toxicity.
